3717-1-06.1(M) / Outer openings - protected.
Repeat Outer back door without proper protection against the entry of insects and rodents (not self-closing).

3717-1-04.3(C) / Fixed equipment installation: elevation or sealing.
Repeat Equipment not properly elevated or sealed to floor. Observed shelving racks with the bottom shelves installed directly on the floor in the walk-in freezer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and other debris on the following nonfood-contact surfaces of equipment: 1) Shelving in walk-in refrigeration and freezer. 2) Walls under the condensor and behind the shelving unit in the walk-in refigeration unit. 3) Gaskets and handles for multiple freezer and refrigeration units.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed damaged paint and surfaces on the exterior of the walk-in refrigeration unit and walk-in freezer.

This facility has a limitation on its license: Operation has not been provided with a properly installed Food Processing sink. All food shall be properly washed, prepared, and provided with a date of preparation at the supplier before being delivered to this location.

Personal belongings shall be kept in designated locations away from the operations products.
Ensure stainless filters above the conveyor cooking equipment is being maintained more frequently than the other filters.
Discussed COVID-19 protocols (face coverings, social distancing, cleaning and sanitizing, sick policy, etc.) during the inspection with the person-in-charge.
The dining room was currently closed and the FSO was offering drive-thru only. Please ensure guidance document stating mandatory and best practices are implemented prior to opening for dine-in.

The violations documented shall be corrected and will be checked during next standard inspection.
