3717-1-02.4(C)(14)/PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Person in charge did not ensure their employees are properly trained in food safety (sick policy, date marking, allergy information on labels), etc.). Education was provided to improve food safety.
3717-1-02.4(C)(15)/PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Employees are not informed in a verifiable manner of their responsibility to report information about their health. Education was provided to improve employee sick policies.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Refrigerated, ready-to-eat, TCS foods not properly date marked. Lettuce and spinach were not date marked in the preparation cold holding unit. Education was provided to improve date marking procedures.

Ready-to-eat, TCS food not properly discarded when required (ham prepared 4/23, chicken prepared 4/24, steak prepared 4/16, eggs prepared 4/26). The PIC voluntarily discarded the items once the violation was identified and education was provided to the staff to improve date marking procedures going forward.

Missing and/or incomplete allergen declarations. The pre-packaged pita chips did not contain a proper label (ingredients, allergens, address, etc.). Education was provided to the PIC about proper labels.
