Working food containers not properly labeled throughout the facility.

3717-1-03.2(K) / In-use utensils - between-use storage.
n-use utensils improperly stored. Observed rice utensil being stored in a container of room temperature water.

3717-1-04.4(P) / Warewashing equipment - determining chemical sanitizer concentration.
Sanitizer test kit or other device not being used. Observed sanitizer hose disconnected from mixing dispenser above the three-compartment sink.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed accumulation of food residue and debris on the rice cookers, shelving units, and on the door handles/gaskets and lids of the refrigeration and freezer unit.

3717-1-06.2(H) / Toilet tissue - availability.
No toilet tissue at each toilet.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors, walls, and area around the grills and where the pizza oven used to be are not maintained clean. Observed a build-up of grease in this area. Ventilation hood system were observed with an accumulation of grease and is in need of cleaning.
