3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed the Hussmann equipment not maintained in good working order at the time of inspection. Frozen foods were not being maintained frozen.
To prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair the Hussmann unit to be in good working order.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Observed accumulation of food residue or dust on nonfood-contact surfaces of the microwave and the condensor for the walk-in at the time of inspection.
To prevent contamination, nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. The equipment listed above shall be cleaned and maintained.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris in the following locations at the time of inspection: (1) The floors in the walk-in (under shelves). (2) The floors under and around pizza preparation cold holding unit/oven. (3) The floors around the bottled drink display cases (center isle).
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the areas listed above.
3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Repeat Observed plumbing fixture for the warewashing sink not maintained in good repair (leaking) at the time of inspection.
To prevent health hazards, plumbing fixtures shall be cleaned as often as necessary to keep them clean and maintained. Repair plumbing fixture for the sink listed above.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary or nonfunctional items in the back storage room and maintenance closet at time of inspection.
To allow for proper cleaning and prevent pest harborage, remove the nonfunctional items from the rooms listed above. The premises shall be free of Items that are unnecessary to the operation or maintenance of the RFE such as equipment that is nonfunctional or no longer used.

Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events.
To prevent health hazards, Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-03.4(A)/Frozen food - temperature and time control.
Observed foods in frozen storage (hussman equipment) exhibit signs of thawing at the time of inspection.
To prevent health hazards, stored frozen foods shall be maintained frozen. Discontinue storing frozen food in Hussman unit untill it has been repaired to function properly.
3717-1-04.1(Y)/Temperature measuring devices.
Observed cold holding equipment (display cold holding) that did not contain a working thermometer, or the thermometer was not correctly placed at the time of inspection.
To ensure equipment is functioning properly, a properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit. Provide a thermometer for the unit listed above.
3717-1-05.4(P)/Maintaining refuse areas and enclosures.
Observed an improperly cleaned storage area for refuse, recyclables, or returnables. Trash was overflowing in the outside receptacle at the time of inspection.
To prevent health hazards, a storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items. PIC stated the trash company comes once a week on Friday. Removal shall be move frequent if trash is found overflowing often.
3717-1-06.4(A)/Repairing.
The physical facilities are not being maintained in good repair. Observed several damaged/stained ceiling tiles in the RFE at time of inspection.
To meet the requirements of this rule, replace the damaged/stained ceiling tiles. The physical facilities shall be maintained in good repair.
Comments
Observed no risk level 3 activities at the time of inspection. There was no food preparation and only pre-packaged foods were being sold.
All risk level 3 and risk level 4 operations shall obtain a level 2 ODH certification in food safety training for one operator of the facility.
Per discussion with new manager, the facility may open up the pizza area in the future. The RFE shall submit plans and contact the health department when this occurs.
The remaining violations documented on this report will be checked during next standard inspection.
