Comments
In order for a FSO (Food Service Operation) License to be issued, the following must be completed:
1) Non-commercial Microwave and Refrigerator shall be removed and replaced with commercial grade equipment. If the equipment is to maintain at the facility and be used for employees, label the equipment "Employee Use Only".
2) Properly install the FRP to be completely sealed to the wall. Painted concrete lock is acceptable if the wall block is smooth, durable and easily cleanable. FRP is not required if the concrete wall blocks meet this requirement. 3 compartment sink shall be properly sealed to the wall.
3) Remove the carpet in the preparation area. Install proper floor tile that can be easily cleaned.
4) Seal the concrete floor under the 3 compartment sink.
5) Repair the leaking drainage line from the 3 compartment sink.
6) Provide an approved sanitizer and proper testing kit. Recommend using Quaternary sanitizers (Steramine or sani-tabs).
7) Provide shatter proof lighting on light above the ordering counter. Switch the light bulbs from the customers area with the lights over the preparation area to provide enough lighting (50 foot candles) to meet code.
8) Ceiling tiles shall be smooth, durable, and easily cleanable over the tea preparation area.
9) Provide a proper sneeze guard around the preparation counter.
10) Rest-room doors shall be self-closing and Self-latching.
11) Floors in the mop room shall be smooth, durable and easily cleanable.
12) Tea preparation counter shall be caulked (create a coved finish), provided with a smooth finish on the inside, and all exposed wood shall be sealed with a polyurethane to water proper the untreated wood.
13) Dumpster shall be stored on a paved surface.
14) Provide documentation of Person In Charge Training for one individual employed. At least one person on each shift shall have this training. By March 1, 2017, at least one person shall be certified in Food Protection by ODH.
15) Provide a Diarrheal and vomiting procedure for review by the Health department.
16) Provide a copy of the Certificate of Occupancy for the facility.
17) Signed application (will be provided once a final approval has been giving) and payment for the amount of $263.00.
