[?] A summary of the violations found during the inspection are listed below.

3717-1-03.5(C)(2) / Food labels - packaged food
Observed no labels on the in-house packaged food items in the display freezers at the dining area. PIC stated labels are in the back and are put on the packaged products when purchased.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer back door without proper protection against the entry of insects and rodents (not self-closing).

3717-1-04.3(C) / Fixed equipment installation: elevation or sealing.
Repeat Equipment not properly elevated or sealed to floor. Observed shelving racks with the bottom shelves installed directly on the floor in the walk-in freezer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and other debris on the following nonfood-contact surfaces of equipment: 1) Shelving in walk-in refrigeration and freezer. 2) Walls in the walk-in refrigeration unit. 3) Gaskets and handles for multiple freezer and refrigeration units.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Plumbing system not properly maintained. Observed water leaking from main hand sink at the time of inspection

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed damaged paint and surfaces on the exterior of the walk-in refrigeration unit and walk-in freezer.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained clean above the burger cooking equipment at the time of inspection. Observed grease build up. Ensure stainless steel filters are being maintained clean and free of grease build-up. Observed dust and debris building up around the intake and exhaust vent covers on the ceiling.

TCS foods require date marks if not used within 24 hours. Please ensure items that are not being used within 24 hours are being properly dated.
Ensure frozen smoothie mixes are being date marked as specified on packaging.
Personal belongings shall be kept in designated locations away from the operations products.
Ensure spray bottles are labeled properly.
Foods stored in the walk-ins or equipment shall be be stored properly and shall not be stored uncovered.

This facility has a limitation on its license: Operation has not been provided with a properly installed Food Processing sink. All food shall be properly washed, prepared, and provided with a date of preparation at the supplier before being delivered to this location.

The violations documented shall be corrected and will be checked during next standard inspection.
