3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Critical Non-food contact surfaces of equipment are dirty. Observed a dark residue accumulated on the interior of the ice machine door. Observed food residue in the gaskets of the reach in cooler. The presence of food debris or dirt on non-food contact surfaces may provide a suitable environment for the growth of harmful organisms.which employees may inadvertently transfer to food.
Nonfood-contact surfaces of equipment shall be kept clean. To reduce the risk of transferring harmful organisms to food, clean these areas, and maintain a regular cleaning schedule to keep them clean.

3717-1-04.1(L) / Ambient air and water temperature measuring devices - accuracy.
Thermometers are not accurate to plus or minus 1.5 degrees C, or plus or minus three degrees F in the intended range of use. Observed hanging thermometer in reach in cooler was measuring the ambient temperature of the reach in cooler at well below freezing, a difference greater than 10 degrees Fahrenheit.
Temperature measuring devices that are scaled in Celsius or dually scaled in Celsius and Fahrenheit shall be designed to be easily readable and accurate to plus or minus 1.5 degrees Celsius or that are scaled only in Fahrenheit shall be accurate to plus or minus 3 degrees Fahrenheit. Obtain a measuring device for the reach in cooler that accurately measures the ambient temperature of the reach in cooler.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed door gaskets to reach in cooler were pulled away from the doors with visible gapping. Door gaskets must be kept sealed to ensure that the reach in cooler continues to function properly.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Repair door gaskets to the reach in cooler so that they seal properly.
3717-1-05.1(E) / Backflow prevention device - design standard.
Observed no backflow prevention device installed on a water supply system. Floor cleaning solution is directly connected to the water supply in the utility sink through a hose that is not protected against backflow and could back siphon into the water supply.
A backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (ASSE) standards and as referenced in the Ohio plumbing code for installation, maintenance, inspection, and testing. To protect the water supply from potential backsiphonage contamination, install an approved backflow prevention device.

Standard and complaint inspection conducted in response to Complaint on 30-Jul-2013 in which complainant states an individually portioned package of applesauce sourced from Giant Eagle and received by this operation caused illness. :
Operation does not provide applesauce in individual portion containers, and no food in the operation is currently received from Giant Eagle. Complainant receives food via operation's Pantry Program. The Pantry Program provides packaged meat from Edinburgh for this program that is received and distributed in a frozen state. The Pantry Program also provides packaged hot dogs and canned goods to recipients.
