3717-1-04.1(KK) / Food equipment - certification and classification.
Observed noncommercial equipment (kitchen aid mixer, coffee machine) that was not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Replace unapproved equipment with a commercial grade (NSF or equivalent testing agency) or remove from facility.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed accumulation of soil residue on multiple nonfood-contact surfaces (storage containers, storage shelving, etc.) at time of inspection.
To prevent contamination, nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean storage containers and shelving equipment. Recommend developing a cleaning schedule to ensure the facility and equipment is maintained clean.
3717-1-05.1(I) / Service sink - number.
Repeat Observed no service sink or curbed cleaning facility at the time of inspection as required.
For disposal of mop water and similiar liquid waste, at least one service sink or one curbed cleaning facility equipped with a floor drain shall be provided and conveniently located.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris on the floors throughout the facility at the time of inspection.
To prevent pest infestation, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. The floors throughout the facility shall be cleaned and maintained.
3717-1-06.4(M) / Storing maintenance tools.
Repeat Observed multiple maintenance tools improperly stored next to food equipment and on food preparation surfaces at time of inspection.
To prevent contamination, maintenance tools shall be stored so they do not contaminate food, equipment, utensils, linens, single-service articles, or single-use articles, and in an orderly manner that facilitates cleaning the area used for storing the maintenance tools. Designate a location for proper storage of maintenance tools.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items throughout the RFE at the time of inspection.
To prevent pest harborage, the premises shall be free of Items that are unnecessary to the operation or maintenance of the RFE such as equipment that is nonfunctional or no longer used. Remove any unnecessary items from the facility to meet the requirements of this rule.
3717-1-07.1(E) / Poisonous or toxic materials: Sanitizers - criteria.
Observed the use of a chemical sanitizer that is not approved for use on food contact surfaces (not NSF approved) at the time of inspection.
To prevent health hazards, chemical sanitizers, including sanitizing solutions generated on site, and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940. Provide sanitizer that is approved for use in a RFE and that state proper food contact surface sanitizing instructions.

Food preparation areas shall be clean and clear of maintenance tools and unnecessary items.
Facility is very limited with space. Recommend additional shelving for better organization.
A level 2 Ohio Department Health certification in food safety is required to be obtained for one operator of the facility by March 1, 2017.
