3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or single-use articles not displayed or dispensed properly. Observed cups removed from original packaging and exposed near the drink dispensing area.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed ice build-up throughout the prep refrigeration unit.

3717-1-04.2(C) / Drainboards.
Insufficient storage space provided for soiled and clean items.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed an accumulation of debris on the gaskets of the prep refrigeration unit. Observed unknown substance around the facet spout and a clear pipe connected to the three-compartment sink faucet.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items on premises. Observed maintenance tools under the hand sink and a coffee brewing equipment on the warewashing machine no longer being used.

Comments
A food defense plan is required to ensure the FSO is protected. Plan shall include details on how the staff will ensure the protection of the FSO when not in operation and the Architectural building is open to the public. Plans for set-up and closure that cover the steps the staff with take to ensure the safety is needed.

Ensure the facility has a manager with a manager level certification in food protection (not to be used for other locations).
Each shift is required to have at least one person with a PIC (person-in-charge) certification.
Employees shall be informed in a verifiable manner of their responsibility to report information about their health (sick policy).
A written procedure for responding to vomiting or diarrheal events shall be readily available.
If water filter are used for water supply lines they shall be maintained and serviced as stated by the manufacturer, documentation shall be readily available.
Post a ODH choking poster at the location.
Post original license at the location.

The violations documented shall be corrected and will be checked during next standard inspection.
