[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at time of inspection.
To prevent health hazards, obtain the ODH level 2 certification in food protection. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Observed a handwashing sink without water at the required temperature. The hand washing sink in the food preparation area only reached 68 degrees F at time of inspection.
To ensure proper cleaning of hands, a handwashing sink shall be equipped to provide water at a temperature of at least 100° F (thirty-eight degrees Celsius) through a mixing valve or combination faucet.

Proper hot water shall be provided at each sink. The hand washing sinks shall be provided with at least 100 degrees F and the ware washing sink (3 compartment) shall be provided with at least 120 -140 Degrees F. A larger hot water tank may be needed or a separate hot water tank for only the FSO supply or even instant hot water equipment installed for the hand washing sink and 3 compartment sink may be necessary.

Obtain a processing license from the OH Department of Agriculture, in order for the food to be processed and prepared at the Main Pizza Shop Angelo's Pizzeria in Rootstown, OH, RFE License # 381. Owners are currently in the process of applying and receiving approval from ODA for the Commercial Processor License. Provide with licensing/permit information once received from ODA to the Kent Health Department.
