[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated, properly installed sink at the time of inspection for preparing ready to eat foods.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with an indirectly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle School kitchen.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at the time of inspection.
To meet the requirements of this rule, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. Provide a level 2 ODH certification in food protection for a proper employee of the FSO.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Observed the person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. PIC stated custodial service would handle all events.
To meet the requirements of this rule, person in charge shall ensure that the FSO have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces. Written procedure shall be developed and implemented in the FSO.

All temperatures measured were acceptable at the time of inspection.
A procedure shall be developed and implemented for TCS foods being held cold on ice. PIC stated TCS foods that are held on ice are discarded at the end on the lunch periods. Lunch period lasts less than 2 hours. Please provide a procedure on this and have it available within the facility.
PIC demonstrated great knowledge on food safety. Facility appeared to be very clean and well maintained.
The violations documented on this report shall be corrected as soon as possible and will be checked during next standard inspection.
