Comments
This inspection was completed in response to the request from the contractor. In order for approval to be issued a Food Service Operation License, the following must be completed:
1) Provide a waste container in women's restroom with a lid.
2) Provide a waste container in the men's restroom.
3) Provide an outdoor refuse storage container.
4) Provide proper handwashing signage, waste containers, soap and means to dry hands at each designated hand washing sink (both rest-rooms, back warewashing area, and two on the main service line).
5) Ensure the front service hand washing sinks fixtures are properly installed to provide 100 degrees F whenever used for hand washing.
6) Provide properly located temperature measuring devices with each reach-in cold holding unit. Each cold holding unit has a temperature measuring device installed on the back wall which is not the correct location per Ohio food code.
7) The front line display case is not easily movable, not installed 6 inches above the floor and is not sealed to the floor. This makes maintaining the floors under the equipment hard to reach and not easily cleanable. Recommend correcting this by installing the equipment higher off the floor to allow for easy access under equipment or install casters to make the equipment movable. Manager stated the floors are part of a daily cleaning checklist and the floors under the unmovable equipment will be checked daily.
8) Provide a proper metal stem temperature measuring device.
9) Provide a maximum registering temperature measuring device or tape/sticker for proper monitoring of high temperature dish machine.
10) Adjust front door to FSO so space between doors is eliminated to reduce pest entry.
11) Provided a signed application for FSO license JSMH-AXNNAD (to be provided at final pre-licensing inspection) and payment for the amount of $500 (made out to Kent City Health Department, due before license may be issued).
12) Provide a copy of the final building/final inspection approving the the facility to be occupied. A certificate of occupancy shall be provided to the Kent City Health Department once provided by the building department.
13) Provide proper back flow prevention device before water conditioning equipment (before the pre-filtration unit).
14) Provide drain plugs in the base of the outdoor refuse storage containers (dumpsters).

This is not a final/complete list and is subject to change upon additional review of the facility. Pre-licensing inspection has been scheduled for 11/1/2018 at 8:00 PM.
