Repeat Handwashing sink(s) are not conveniently located for the warewashing area in back room. There is not a designated hand washing sink in the warewashing area.

Repeat Observed multiple equipment with components are not intact, tight or properly adjusted. Multiple reach-in coolers were not longer in proper working order (not in use). Remove or repair equipment that is no longer being used (reach-in display coolers, freezers, ice machine, deli slicer, pizza oven, etc).

Repeat Observed multiple equipment items that are no longer necessary to the operation. Remove the Hunts brothers, reach- cold holding units, pizza oven, deli display case, deli slicer, air fryer, grill, ice machine, racks, and any other equipment that is not needed for the day to day operation. Unnecessary or nonfunctional items and /or litter on premises.

Repeat Observed floors under walk-in cold holding unit to be in need of cleaning. Under display racks, under the beverage delivery racks (shall be removed to allow for easiler cleaning). Facility not maintained clean.

Repeat Unapproved materials covering the walls and/or ceiling in the ware washing back room. Exposed concrete block shall be covered or sealed to be smooth, durable and easily cleanable.

Repeat Observed loose litter around the Outdoor refuse container location, parking lot and grassy area behind the building. Unnecessary or nonfunctional items and /or litter on premises.

PIC unable to demonstrate knowledge by having no critical violations.

Handwashing sign was not posted at the both hand washing sinks. Provide a hand washing sign.

No soap at handwashing sink rest room or front counter hand washing sink.

No towels or drying device at both of the handwashing sinks. Provide a means to dry hands according to food code.

Coffee brewing utensils and equipment were not cleaned at required frequency (24 hours/daily). Due to the condition of the warewashing facilities, ware washing cannot occur correctly.

Warewashing sink not properly cleaned and sanitized. Observed the warewashing sink to be dirty and in need of cleaning and sanitizing before use.

Non-food contact surfaces of equipment are unclean. Observed handles, door seals and door frames to reach-in cold and freezer display equipment (walk-in display doors and reach-in display doors).

No drain plugs in exterior waste receptacle.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed floors in the retail store to be in need of cleaning. Under and behind equipment was in need of cleaning. Facility not maintained clean.
3717-1-06.4(N)/Maintaining premises - unnecessary items and litter.
Observed multiple cans of paint, chemicals for cleaning, personal care items that are unnecessary daily operations of the RFE.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed ceiling air intake vent to be in need of cleaning at the time of inspection. Facility not maintained clean.
Comments
Observed significant improvements that corrected many of the violations from previous report.

RFE has drop to a Risk Level II RFE and will no longer provide prepared foods, coffee and any other risk level III items. Due to a lack of proper warewashing facilities, the RFE is limited until the warewashing and handwashing facilities improve. Improvement will require a plan review with licensor.

OAC Rule 3701-21-02.3 | Risk level of food service operations.
"(B) Risk level II poses a higher potential risk to the public than risk level I because of hand contact or employee health concerns but minimal possibility of pathogenic growth exists. Examples of risk level II activities include, but are not limited to:
(1) Handling, heat treating, or preparing non-time/temperature controlled for safety food;
(2) Holding for sale or serving time/temperature controlled for safety food at the same proper holding temperature at which it was received; or
(3) Heating individually packaged, commercially processed time/temperature controlled for safety foods for immediate service."

Contact the KCHD (licensor) when there a plans to begin providing TCS food menu items. KCHD shall be notified if there are changes to facility or Menu options (that may increase Risk).

At this time, a follow-up inspection will not be required. Continue to maintain and to make improvements to avoid enforcement action from the Licensor. It is recommended that communication regarding improvements with licensor to ensure compliance with the Ohio Food Code and rules.

All violations on this report will be rechecked at the next standard inspection.
