3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Corrected During Inspection Observed employee drinking from an unapproved beverage container at the time of the inspection.
To prevent contamination, an employee shall drink from an approved container that contains a tight fitting lid.
3717-1-02.3(C) / Hair restraints - effectiveness.
Observed employees that did not have their facial hair effectively restrained at the time of inspection.
To prevent contamination, food employees shall effectively restrain or cover facial hair to keep their hair from contacting exposed food, clean equipment, utensils or linens or unwrapped single-service or single-use articles. Beard snoods are recommended.
3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper use of wet wiping cloth (underneath cutting board for anti slip mat) at the time of inspection.
To prevent contamination, cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose. Discontinue use of wet cloths as anti slipping mats for under cutting boards.
3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Corrected During Inspection Observed can opener blade creating metal fragments and dirty at the time of inspection.
To prevent contamination, cutting or piercing parts of can openers shall be kept sharp to minimize the creation of metal fragments that can contaminate food when the container is opened. The can opener shall be cleaned as often to remain clean.
3717-1-06.1(L) / Toilet rooms - enclosed.
Observed the employee restroom not completely enclosed and self-closing door at the time of inspection.
To prevent health hazards, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. Repair self closing mechanism to allow door to tightly close on its own.
3717-1-06.4(J) / Using dressing rooms and lockers.
Corrected During Inspection Observed an employee jacket on the food preparation sink. Employee personal belongs were not being properly stored at the time of inspection.
To prevent contamination, personal items shall be stored in a designated area. Remove jacket from on top of the food preparation sink and store all employee personal belongs in the proper designated location.
901:3-4-7.0 / Facility layout and equipment specifications for RFE
Observed RFE that was altered, and equipment was added without prior approval from the licensor. Observed a wall was removed (opening the preparation area to customer contamination), new pizza oven, kitchen equipment layout had been altered at the time of inspection.
To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a retail food establishment until the facility layout and equipment specifications therefore have been submitted to and approved in writing by the licensor. The facility layout and equipment specifications shall be approved by the licensor in accordance with the rules adopted pursuant to section 3717.05 of the Revised Code stating the criteria for facility layout and equipment specifications. Provide the make and model number of the oven and the new layout for the operation. The wall that was removed shall be replaced or facility shall be altered to ensure preparation areas are protected from customer contamination.
