3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. Raw sausage was stored over RTE (cheese, ham, and vegetables) at time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked at time of inspection (chicken, ham, tomatoes, meatballs, etc.).
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.

3717-1-04.8(F) / Storage - prohibitions.
Corrected During Inspection Cleaned and sanitized equipment, utensils, laundered linens, or single-service articles or single-use articles were stored in a prohibited area. Observed cleaned towels stored in the restroom at time of inspection.
Equipment, utensils, laundered linens, or single-service articles or single-use articles may not be stored in the areas specified under this rule where contamination might occur.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom at time of inspection.
To prevent health hazards, provide a lid for the trash can in the restroom. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening, window, or door without proper protection against the entry of insects and rodents at time of inspection (front door).
To prevent contamination, provide a proper screen door with a self-closing device and/or discontinue the practice of propping open the current door. Outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed multiple light bulbs in the kitchen not working at time of inspection.
To prevent health hazards, replace the non-working light bulbs. The physical facilities shall be maintained in good repair.
