Critical Observed flying insects near the warewashing area and ront hand sink. Ensure buckets of mop water is being maintained, floor drains are cleaned, and general cleaning in the areas are increased.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer back door without proper protection against the entry of insects and rodents (not self-closing).

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed lid stored in the sprinkle containers being used as a scoop.

3717-1-03.2(N)(4) / Gloves - latex gloves
Latex gloves being used in the FSO.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and other debris on the following nonfood-contact surfaces of equipment: 1) Shelving in walk-in refrigeration. 2) Walls under the condensor and behind the shelving unit in the walk-in refigeration unit. 3) Gaskets and handles for multiple freezer and refrigeration units (damaged gaskets shall be repaired).

Plumbing system not properly maintained or repaired. Observed a bucket collecting water under the three-compartment sink.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility.

Operation has not been provided with a properly installed Food Processing sink. All food shall be properly washed, prepared, and provided with a date of preparation at the supplier before being delivered to this location.

Observed black tape across the ceiling in the back storage area with staining and evidence of water leak in the corner.
The walk-in light intensity shall be 10 foot candles during cleaning. Additional lighting may be needed.

The violations documented shall be corrected and will be checked during next standard inspection.
