3717-1-05.1(Q) / Scheduling inspection and service for a water system device.
Repeat Observed a water treatment device or backflow preventer is not inspected or serviced as required at time of inspection (ice machine filter was last serviced 4/26/16).
To prevent health hazards, a device such as a water treatment device or backflow preventer shall be inspected and serviced in accordance with manufacturer's instructions and as necessary to prevent device failure, and records demonstrating inspection and service shall be maintained by the person in charge.
3717-1-06.2(I)(1) / Lighting - intensity (10)
The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. Observed the mop room with less than ten foot candles at time of inspection.
To prevent health hazards, repair the light. The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
3717-1-06.2(I)(3) / Lighting - intensity (50)
The light intensity in an area where an employee was working with food, utensils or equipment was less than fifty foot candles. Observed the area near the food preparation sink with less than 20 foot candles at time of inspection.
To prevent health hazards, repair the light. The light intensity shall be at least fifty foot candles (five hundred forty lux) at a surface where a food employee is working with food, utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed multiple areas of the building in need of repair at time of inspection. The main hand washing sink had a leaking faucet fixture. There were several damaged ceiling tiles in the dining room. Observed multiple non-working light bulbs in the preparation area and the mop room at time of inspection.
To prevent health hazards, repair the FSO. The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt, dust, grease, and debris along the floors of the preparation area in the hard to reach areas at time of inspection.
To prevent contamination, thoroughly clean the floors. The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
