Comments
This per-licensing inspection was completed per the request of the contractor and manager of the FSO for licensing approval. The FSO has been approved to be licensed with the understanding the following will be completed as soon as possible and within 30 days:
1) Provide a proper back flow prevention device on the direct connect to the filter/water softener equipment or provide documentation form a licensed plumber that the device is not required.
2) Provide maximum registering temperature device or tape/sticker to monitor the high temperature ware washing machine.
3) Adjust or repair the front door to eliminate gap between doors. This is required to reduce pest/insect entry.
4) Provide proper drain plug in both of the outdoor refuse storage containers (dumpsters).
5) Provide a paper towel dispenser at the second hand washing sink next to the turbo chef. Paper towels are provide at the hand washing sink in the warewashing area which is reasonably accessible.
6) Recommend including to have employees check under the front display case/cold holding equipment for cleaning. Recommend installing the equipment on casters or at least 6 inches up off the floor.
7) Provide a proper diarrheal and vomit episode clean-up procedure and clean-up kit (disinfect and PPE). See handout in inspection email.
8) Ensure employees have been trained regarding what symptoms to report to the manager and provide the documentation (paper or digital) for inspector review.
9) Ensure all preparation areas and food contact surfaces are properly cleaned and sanitized before beverages and foods are prepared for sale to the public.
10) Post choking poster somewhere in the facility. See inspection email for a PDF copy.

A signed application and payment Check # 018744 for the amount of $500 was received at the time of inspection for License # JSMH-AXNNAD risk classification III. No Limitations or restrictions have been placed on this license at this time.
Final Building and Fire inspection was provided at the time of inspection.
