Person in charge did not ensure their employees are properly trained in food safety. The PIC did not have adequate food safety knowledge (sick policy, vomit cleanup procedure, hand washing, etc.). Education was provided to improve food safety.

Employees are not informed in a verifiable manner of their responsibility to report information about their health. The PIC was educated about the proper signed sick policy that is required for all employees.

FSO did not submit facility layout and equipment specifications. The FSO made extensive alterations without prior approval from the Kent City Health Department. A Facility Review is required before alterations can be made to an FSO. There is a new wall, hand washing sink, and dipper well constructed along the renovated front counter area.
