Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (Ice machine and ice bins behind all bars).
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To protect ice from contamination, eliminate all direct connections to floor drains.Plumbing alterations should be made only by a licensed plumber. Correct By: 10-Apr-2014

At the time of inspection, observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.4(A) / Repairing.
At the time of inspection, observed the laminate on the corner of the downstairs bar was peeling.
Repair the downstairs bar surface so that is smooth, durable, and easily cleanable.
