Comments
All violations from previous inspection were observed. No improvements or corrections have been completed.

A new manage was present for the inspection.

Due to the number of repeat violations observed at the time of inspection the RFE will be considered in a state of failure to maintain. Additional enforcement action will be required, if significant efforts are not made to correct items documented on this report and to become compliant with the Ohio Food Code. This inspection was to be considered a Pre-Administrative Meeting Compliance Inspection to ensure that expectations of the licensor have been properly communicated and documented. Due to the RFE being under New Management, the Pre-Administrative Meeting Compliance Inspection will be completed January 5 at 10:00 AM.

Based on the current state of the RFE, it is recommend that the owner requests the RFE risk level to be dropped to a Risk Level II. This limits the RFE to not be able to cook and assemble foods. Due to the number of violations and concerns regarding the sanitation and knowledge, this is strongly recommend to be completed as soon as possible. All pizza and food preparation equipment shall be removed (pizza oven, cooler, grill, air fryer, deli case and deli slicer shall be removed. Please submit the request to drop risk level when ready.

A follow-up inspection has been schedule for 1/5/2022 at 10:00 AM to allow the new manager to address with Health and Fire concerns. If significant effort is not made to comply with the items on this inspection report, enforcement action from the licensor can be expected.
