Comments
The following items shall be completed before the FSO will be approved to be licensed:
1) Provide a complete list of all make and model numbers of all equipment
2) Seal the Frozo Machine (margarita machine) to the bar counter or elevate 4 inches (for cleaning under) or make the machine easily movable.
3) Seal/caulk around the bar counter top and the support beam juncture.
4) Repair damaged coved base around the bar.
5) Caulk or install coved base molding along the floor and bar wall juncture of the front bar
6) Caulk or install coved base molding along the floor and cabinet juncture next to the beer tap cabinet
7) Provide a proper Air Gap on the discharge drain of the bar dish machine.
8) Provide a back flow prevention device on the bar dish washing machine.
9) Provide a proper threshold over the beer tap drainage lines in back closest. Install shall be smooth, durable and easily cleanable.
10) Provide a proper backflow prevention device on beer glass rinse water lines. (one for both)
11) Add additional lighting over the 3 bar compartment sink. At least 20 or 50 foot-candles if knives are to be cleaned in bar sink.
12) Caulk or install coved base molding along the floor and bar wall juncture under the server drink station
13) Recommend not using brick under the ice machine. Research another solution that would be considered smooth, durable and easily cleanable.
14) Caulk and seal any holes left in the walls from old equipment. Seal gaps around plumbing lines and wall junctures.
15) Provide additional lighting at the Prep table/preparation sink area. At least 50 foot candles are required.

17) Seal the 3 compartment sink to the wall.
18) Provide/install a proper mop hanger. Recommend the hanger to be installed over the mop sink.
19) Add additional light bulbs in the mens room. Provide at least 20 foot candles at the hand washing sink.
20) Provide additional lighting in the walk-in beer cooler. 2 foot candles was observed. Provide at least 10.
21) Provide a procedure/policy regarding the future garage door use and pest issues.
22) Per the FDA guidelines, the tankless hot water supply is less than what is required. Recommend providing an additional tankless hot water heater. A hot water supply check will be completed at the next inspection. If issues are observed, licensing may not be issued.
23) Provide documentation that the the grease interceptor was services and properly functioning.
24) Provide documentation that the main backflow prevention device was inspected and is properly functioning.
25) Provide a final building and fire department inspection and Certificate of Occupancy.
