[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical Observed the person in charge was unable to retain the label for the bottled sauces located in the walk-in cold holding unit at the time of inspection.
To prevent factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall retain labels for food items. Provide labels for the sauces.

3717-1-03.5(C)(3) / Food labels - bulk food
Observed bulk food display for customer self-service was not properly labeled at the time of inspection. Bananas did not contain a label with information containing the common name and country of origin.
To prevent health hazards, bulk food that is available for consumer self-dispensing shall be prominently labeled with the information specified under (C)(2) of this rule. Provide a sign or other method of notification that contains the common name and country of origin of the food item.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed the plumbing system for the ice machine not properly maintained at the time of inspection. The drainage lines were leaking water onto the floor.
To prevent health hazards, a plumbing system shall be properly maintained. Repair the plumbing system for the ice machine to be in good working order and directed to a floor drain.
