[?] A summary of the violations found during the inspection are listed below.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Critical Observed employee eating, drinking, or using tobacco in non-designated area or was observed using unapproved beverage container.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result. Only approved beverage containers shall be used.
3717-1-05.3(C) / Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed FSO is not properly supplied with a properly installed food preparation sink with indirect connection to sewer system at time of inspection.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Foods are not to be washed in the 3 compartment ware washing sink or the hand washing sink. Provide a food preparation sink without a direct connection to limit the contamination to ready to eat foods. All foods shall be prepared in a properly supplied kitchen and transported ti this location until a food preparation sink is provide
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Critical Observed no towels or hand drying device at the handwashing sink(s). Observed hand washing sink was not properly provided with a paper towel dispenser at time of inspection.
Each handwashing sink or group of adjacent handwashing sinks shall provide hand drying by a means described in this rule. To promote proper hand washing paper towels shall be provided adjacent to the handwashing sink. Provide a paper towel dispenser in a location that prevent contamination to clean utensils on the air drying rack (don't install the dispenser directly above the air drying rack). Paper towels were available at time of inspection.

3717-1-02.2(G) / Jewelry - prohibition.
Observed food employees wearing jewelry on arms or hands during food preparation.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands.
3717-1-03.2(Y) / Miscellaneous sources of contamination.
Miscellaneous sources of contamination observed. Observed school employees person drinks were being stored in the kitchens main cold holding equipment at time of inspection.
Food shall be protected from contamination that is not specified under 3717-1-03.2 (A) to (X). Only foods and drinks designated for sale to the customer shall be stored in a licensed kitchens refrigeration unit. All personal belongs, foods and beverages shall be stored in a designated location.
3717-1-04.1(KK) / Food equipment - certification and classification.
Cooking equipment is not approved by a recognized testing agency. Observed noncommercial stove/oven range at time of inspection.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-06.1(A) / Floors, walls, and ceilings.
Observed space along base of grease trap was not smooth and easily cleanable.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth, durable and easily cleanable. Caulk along base of grease trap to provide a easily cleanable surface.
