Repeat Facility does not have an employee with manager certification in food protection.

Outer opening not protected. The front door was left propped open at time of inspection.

3717-1-03.2(T) / Food preparation - preventing contamination from the premises
Repeat Food not protected from environmental sources of contamination during preparation. The preparation table is exposed to customers at the front service counter.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors are not clean under equipment in the kitchen. Walls are not clean near the exhaust hood.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Multiple lights do not work in the kitchen and there are multiple damaged ceiling tiles and floor tiles.
