PIC unable to demonstrate knowledge of preventing foodborne illness by controlling contamination, personal hygiene, and facility maintenance. Observed a large number of violations at time of inspection, including critical violations (sick policy and bare hand contact). Education was provided to the PIC to improve food safety procedures.

Improper storage of pizza boxes on the floor of the kitchen and improper storage of clean linens on the shelf of the bathroom.
