3717-1-02.4(C)(14) / PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Critical Person in charge did not ensure their employees are properly trained in food safety. Observed a large number of violations at time of inspection indicating the staff is not properly trained in food safety (chemical labels, sick policy, hand washing sinks, flies, and facility maintenance). Education was provided to the PIC to improve food safety procedures.

Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. Employees have not signed the employee sick policy. Education was provided to the PIC about the proper sick policy requirements.

Critical Working containers of poisonous or toxic materials not properly labeled. Chemical spray bottles were not labeled. Education was provided to PIC about proper chemical labels.

3717-1-06.4(K) / Controlling pests.
Critical Large population of drain flies in the basement. Education was provided to PIC about proper pest control.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Basement ice bin drain does not have proper air gap. Education was provided to PIC about proper drain requirements.

No soap at handwashing sink in basement women's restroom.

Repeat Handwashing sinks are not conveniently located as specified in rule. The entire first floor bar is missing a properly located and working hand washing sink.

Outer opening not protected. Basement door does not self close allowing for potential pest to enter FSO.
