Critical Refrigerated, ready-to-eat, TCS foods not properly date marked (ham, lettuce, and sausage). Education was provided to improve date marking procedures.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items stored on the floor of the walk in freezer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean (cooking equipment, cold holding equipment, pizza oven, freezer, mixer, storage racks and shelving).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple areas of the kitchen with damaged walls, ceilings, lights, and shelving. There are multiple surfaces that are not smooth, durable, and easily cleanable.

Excess litter outside near dumpster, parking area, and near walk-in cold holding unit.
