3717-1-05.1(J) / Backflow prevention device - when required.
Critical Repeat Observed no approved backflow prevention device on the plumbing system for the coffee machine near the warewashing area at the time of inspection.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO. Install a backflow prevention device on the coffee machine.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the ice machine in the concessions area was not installed with a proper air gap at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide a proper air gap on the drainage line from the ice machine to prevent contamination to the ice.

3717-1-03.5(C)(1) / Food labels - packaged food
Repeat Observed multiple food packaged in the facility for customer self-service (veggies,fruit,salads,sandwiches) that did not contain a label at the time of inspection.
To properly inform consumers, foods shall be labeled as specified in this rule. Provide common identity label for packaged food items that did not contain labels.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed the shelving in the walk-in cold holding equipment not maintained in good working order at the time of inspection. Multiple sections were found in poor condition. Standing water was found on the floor.
To prevent contamination, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair or replace the shelving indicated in the observation. Ensure drain pipes are directed into the floor drain.
3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed light bulbs installed in the pizza hot holding unit where exposed pizzas are held for customer self-service that were not properly shielded or coated at the time of inspection.
To prevent contamination, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. Provide proper shielding for the unit indicated in the observation.

3717-1-03.3(H)/Reheating for hot holding.
Observed improper reheating of nacho cheese for hot holding at the time of inspection.
To prevent the growth of pathogens, time and temperature controlled for safety (TCS) food that is cooked, cooled and reheated for hot holding shall be reheated as required by this rule. TCS food that is cooked, cooled and reheated for hot holding shall be reheated to a temperature of at least 165°F for 15 seconds. Hot holding unit shall not be used for reheating unless it is capable of reaching the required temperature.
3717-1-04.1(DD)/Warewashing machines - automatic dispensing of detergents and sanitizers.
Observed warewashing machine that did not incorporate a visual means to verify that detergents and sanitizers are delivered or a visual or audible alarm to signal if the detergents and sanitizers are not delivered to the respective washing and sanitizing cycles. Observed detergent not being dispensed into the warewashing machine during the wash cycle at the time of inspection.
To ensure proper cleaning and sanitization, a warewashing machine installed after March 1, 2005, shall be equipped with a visual means to verify that detergents and sanitizers are delivered or a visual or audible alarm to signal if the detergents and sanitizers are not delivered. PIC stated Ecolab will be contacted to make the necessary repairs. Facility shall warewash in 3-compartment sink until repairs are made.
Comments
Observed an acceptable written procedure for the nacho cheese.
Continue to monitor the warewashing machine to ensure its functioning properly.
The remaining violations shall be corrected as soon as possible and will be checked during next standard inspection.
