Critical Corrected During Inspection Improper use of a poisonous or toxic substance. Unapproved pesticide being used in the basement. The PIC immediately removed the device once the violation was identified.

Equipment not approved by a recognized testing agency. Non-commercial food storage containers are being used in kitchen (gladware, etc.).

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean (cold holding equipment, storage racks and shelving).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Multiple lights are not working. Multiple areas throughout the building with damaged ceiling tiles, floor tiles, damaged coved base, and missing caulk.

Repeat Unnecessary or nonfunctional items stored in basement (excessive amount of old equipment).
