Repeat Observed a water treatment device for the coffee machine not inspected or serviced as required at the time of inspection.
To prevent health hazards, a device such as a water treatment device or backflow preventer shall be inspected and serviced in accordance with manufacturer's instructions and as necessary to prevent device failure, and records demonstrating inspection and service shall be maintained by the person in charge. The water treatment device shall be inspected and serviced to meet the requirements of this rule.

The light intensity in a customer self-service area and inside equipment was less than twenty foot candles at the time of inspection. Observed missing bulb in the cold holding machine and non-working bulbs for both machines at the time of inspection.
The light intensity shall be at least twenty foot candles (two hundred fifteen lux): at a surface where food is provided for customer self-service such as buffets or salad bars or where fresh produce or packaged foods are sold or offered for human consumption; inside equipment such as reach-in and under-counter refrigerators and at a distance of thirty inches (seventy-five centimeters) above the floor in toilet rooms and in areas used for hand washing, warewashing, equipment storage, or utensil storage.
