[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect concentration for the warewashing machine. Observed chlorine measured at 0 PPM. The sanitizing solution needed to be adjusted at the container to be air tight and the line needed to be primed. The PIC stated they have been using the three-compartment sink to clean and sanitize equipment and utensils. Observed warewashing machine in use and shall be maintain if being used.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the ice machine in the concessions area was not installed with a proper air gap at the time of inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed orange cabinet above food prep sink peeling paint and difficult to clean. Observed butcher block prep table unable to be properly cleaned (not used as food contact). Observed wood shelf under the table at the dish washing window.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed ice build-up in the walk-in freezer.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Expired sanitizer test kit available for the warewashing machine.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed debris and possible mouse droppings on top of the warewashing machine (no other locations were found with possible mouse droppings). Observed crescor hot holding cabinet throughout the facility not maintained clean (specifically around the handles).

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors no longer smooth and easily cleanable. Observed damaged floor tiles in front of the walk-in and around the warewashing machine.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed peeling paint, black substance and cracks on the ceiling in the walk-in refrigeration unit. Walls and ceilings in the walk-in are not smooth and easily cleanable.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat No protective shielding on lights in the walk-in refrigeration unit.

The violations documented shall be corrected and will be checked during next standard inspection.
Light intensity in the walk-in freezer shall be 10 foot candles. Additional lighting may be needed.
Observed the middle door on the 3-door refrigeration unit appeared to not seal properly when other doors are used.

VI - P - TCS Food: Observed ready to eat TCS foods being properly date marked, and discarded when required.
VII - P - Protection from Contamination: Foods are protected from physical and environmental contamination during storage, preparation, holding and display.
IV - P - Demonstration of Knowledge: The person in charge is Certified in Food Protection.
