3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (2 small household refrigerators) at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Remove this unapproved equipment.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Corrected During Inspection Observed cleaned equipment (beverage containers) were stored with lip-contact surfaces in direct contact with an absorbent surface (linens) at the time of inspection..
Absorbent surfaces, such as linens or towels, that become moist create an environment where harmful bacteria may grow. To prevent contamination, store beverage containers on a clean, dry surface that does not absorb moisture. Linens were removed at the time of inspection.
3717-1-06.2(E) / Handwashing signage.
Repeat Observed no handwashing sign posted at handwashing sink in the men's restroom on the second floor at the time of inspection.
To encourage proper handwashing, a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris on floors near refrigerated equipment on the first floor at the time of inspection.
To eliminate sources of contamination that may attract pests, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Provide deep cleaning that removes all dark build-up on floors behind the first floor bar, and remove all debris from the top floor storage area. Work at removing any items that are not being used.

Observed the handwashing sink behind the first floor bar was not functional at the time of inspection.

3717-1-05.3(C)/Backflow prevention.
Observed direct connections at the time of inspection between the sewage system and a drain originating from equipment (ice bin behind 1st floor bar and the 3rd floor ice machine) in which food (ice) is placed.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food is placed. Eliminate these direct connections. Correct By: 02-Apr-2015

Observed working containers of poisonous or toxic chemicals (spray bottles containing liquid) that were not properly labeled as to their contents at the time of inspection.

Upstairs storage area appeared much cleaner at the time of inspection. Continue to make progress in maintaining all areas of the facility clean.
