This Food Service Operation has been approved to be licensed. A signed application for license JSMH-A2MQW9 and check# 1622 for the amount of $238.00 was provided at the time of inspection. Okay to Operate.

The license will be mailed to mailing address provided on the application.

Recommend completing the following:
Caulk along the 3 compartment sink along the sides that are against the wall.
All cold holding equipment shall be on casters or installed to be at least 6 inches above the floor.
Only NSF or equivalent equipment and utensils will be permitted to be used. Serve-One equipment will not be approved unless proof of approval can be provided.
A means to separate the public area from the cafe preparation area is required.
Sneeze guards may be required in the future to protect the preparation area from customer contamination. This will be reviewed closer once in operation.
