[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (food preparation sink) in which foods are placed or washed at the time of inspection.
To prevent contamination to foods and equipment, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless. Provide proper back flow prevention on the sink used for washing ready to eat foods. Until this change is made, all food shall be washed at the commissary kitchen (Middle School).

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed food containers that were not approved by a recognized testing agency were used for serving lunches at the time of inspection.
To prevent contamination to foods, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. The current containers being used are not approved for use in a licensed FSO but may be used until the container is no longer able to be properly ware washed (containers becomes scored, scratched, or damaged). These containers need to be phased out and replaced with NSF approved (or similar testing agency).
3717-1-06.2(D) / Handwashing aids and devices - use restrictions.
Repeat Corrected During Inspection Observed soap and paper towels for handwashing at the utensil washing sink (3 compartment) at the time of inspection.
A sink used for food preparation or utensil washing, or a service sink or curbed cleaning facility used for the disposal of mop water or similar wastes, may not be provided with handwashing aids and devices required for a handwashing sink. Recommend installing a paper towel dispenser by the designated hand washing sink to promote proper hand washing by food employees. Paper towel dispenser and soap dispenser were removed from the wall by the warewashing sink at the time of inspection.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items (food processor, crock pot, hand mixer, and other items not used by this FSO) on the premises at the time of inspection.
To allow the storage room to be easily cleaned, the premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO such as equipment that is nonfunctional or no longer used and litter. Remove all items from the storage room that are not used by this FSO.
