Comments
This FSO has undergone a Facility Review due to new ownership to issue a new license to the new license holder. FSO is approved to be issued a new FSO license with the understanding the following will be completed with in 30 days:
1) All larger Ice Cream freezers shall be made movable by installing casters, elevated to be above the floor 6 inches, or sealed to the floor and wall.
2) All doors that lead to the outside shall be made self closing and self latching.
3) Repair all peeling or chipping paint through out the facility. Drop ceiling frame, drywall ceiling, etc.
4) Seal hand washing sink in rest room to wall.
5) Seal 3 compartment sink to wall.
6) Repair all crack or damaged floor tiles through out FSO.
7) Ensure that the ventilation fan in rest room vents to the outside.
8) Coke cooler shall not be used for storing opened packages of foods, toppings etc.
9) Submit a complete facility review application will all required documents and review fee.
10) Ensure all equipment with direct water connections are provided with proper back flow prevention devices or air gaps.
*11) Until a properly installed food preparation sink can be installed, this FSO will be restricted to only use ready to eat foods (fruits) that have been have been properly washed by the supplier and protected from contamination. This restriction will be included on the license.

FSO is approved to operate once the above items have been completed and a signed application and payment for new license is submitted to the Kent health department.
