3717-1-04.8(F) / Storage - prohibitions.
Repeat Single-service articles or single-use articles were stored less then 6 inches above the floor at time of inspection.
Equipment, utensils, laundered linens, or single-service articles or single-use articles may not be stored in the areas specified under this rule where contamination might occur.
3717-1-06.1(L) / Toilet rooms - enclosed.
Repeat A toilet room was not completely enclosed and/or did not have a tight-fitting, self-closing door.
A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.

The following violation(s) have been corrected since the last inspection.
3717-1-03.2(K)/In-use utensils - between-use storage.
Observed raw chicken in-use utensil was improperly stored with handle in direct contact with raw chicken.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule. In-use utensils shall be stored with their handles above the top of the food and the container.
3717-1-07.1(A)/Storage: separation.
Observed improper storage of poisonous or toxic materials over and around single service items at time of inspection.
Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles.
3717-1-06.4(M)/Storing maintenance tools.
Observed maintenance tools improperly stored which resulted in contamination.
Maintenance tools shall be stored so they do not contaminate food, equipment, utensils, linens, single-service articles, or single-use articles, and in an orderly manner that facilitates cleaning the area used for storing the maintenance tools.
3717-1-06.4(J)/Using dressing rooms and lockers.
Observed designated closest was not being properly used by employees to store personal items at time of inspection. Observed employees purse in kitchen on reach-in freezer and then moved to food preparation tables.
Suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Employee personal belongs are not permitted to be out on food preparation counters or behind the serving line.
3717-1-04.4(S)(1)/Single-service articles and single-use articles - use limitation.
Observed re-use of single-service (cheese packaging containers) or single-use articles. Observed foods were being left in opened cans.
Single-service and single-use articles may not be reused. Containers which food has been delivered in shall not be re-used for storage or preparation of other foods. Once a can has been opened, foods shall be transported into a approved container.
3717-1-04.5(J)/Rinsing procedures.
Utensils and/or equipment were not properly rinsed.
Washed utensils and equipment shall be rinsed so that abrasives and cleaning chemicals are removed or diluted through the use of water by using one of the procedures specified in this code. Utensils shall be pre-cleaned, washed, rinsed, the sanitized specified in this code. Provide a proper procedure for the ware washing process at this facility.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Refrigerated, ready-to-eat, TCS foods (opened hot dog package, opened feta cheese, cut leafy greens) held refrigerated for more than 24 hours were not properly date marked.
Refrigerated, ready-to-eat, TCS food held refrigerated for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day to be consumed, sold, or discarded as required in this rule.
3717-1-02.2(G)/Jewelry - prohibition.
Observed food employees wearing jewelry on arms or hands during food preparation.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands.
3717-1-02.3(A)/Food contamination prevention - eating, drinking, or using tobacco.
Observed employee drinking in non-designated area out of a unapproved beverage container at time of inspection.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result. Only approved beverage containers shall be used.

Observed the Kitchen, Dinning area and Front service area to be very clean at the time of inspection.
