3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Food employee arriving to work did not wash hands when required. Handwashing shall occur prior to donning gloves after changing tasks.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection.

3717-1-06.2(B) / Handwashing cleanser - availability.
No soap at handwashing sink in kitchen. PIC placed dish soap at the sink and stated hand soap will be purchased.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Food thermometer not readily accessible.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed damaged/missing weather stripping and seals on the front entrance door.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw vegetables washed improperly. Vegetables are being washed after being cut and prepared. Ensure vegetables are being washed thoroughly under running water in the food prep sink before cutting and preparing the products.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed grill utensils and rice utensil being stored in a container of room temperature water.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue, grease, and other debris on the gaskets, handles and doors of the prep refrigeration unit and equipment on the cook line.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Plumbing system not properly maintained or repaired. Observed sewage-like odor in the restroom at the time of inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors throughout especially along the baseboards by the three-compartment sink, grills, and the pizza oven are not maintained clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items on premises Observed non-functional equipment in the main prep room (large dough mixer, pressure cooker, and reach-in cold holding unit). PIC stated these items are going to be removed from the landlord.
