3717-1-06.4(K) / Controlling pests.
Critical Observed multiple flies near the warewashing sink (3-compartment sink) at the time of inspection.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (blender, microwave, 2 refrigerators, crock-pot, handheld mixer, 2 roasters) at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Unapproved equipment shall be removed or replaced with commercial equipment.
3717-1-07.1(E) / Poisonous or toxic materials: Sanitizers - criteria.
Observed the chemical sanitizer that was not approved for use on food contact surfaces at the time of inspection.
To prevent health hazards, chemical sanitizers, including sanitizing solutions generated on site, and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940. Provide sanitizer that is approved for use in a FSO and that state proper sanitizing instructions.

The operation was currently only providing pre packaged snacks at the time of inspection.
Breakfast and Lunches were provided by an exempt child food program (Community Action Council of Portage County, Inc). The program receives prepared foods from innovation food service.
A level 2 Ohio Department of Health certification in food safety is required to be obtained for one operator of the facility by March 1, 2017.
