[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed multiple TCS foods (open bags of cut leafy greens, pesto, etc) without proper dark marking at the time of inspection. If foods are TCS, containers are required to be date mark for 7 days only. Reviewed what items in the FSO were TCS with manager, corrections made on site. Recommend using the date of preparation.

Corrected During Inspection Food employees wearing jewelry on arms or hands during food preparation.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Observed cafe beverage cups were removed from original packaging and on display without protection. Single-service and single-use articles not protected from contamination.

3717-1-04.4(Q)(3) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Ambient air temperature measuring device for the display cooler was not accurate (reading 48 degrees F when foods were 41 and below). Recommend arranging shelves and TCS food items in the coldest locations for the cooler.

3717-1-04.3(C) / Fixed equipment installation: elevation or sealing.
Observed the Espresso equipment was installed on the counter without proper spacing or elevation. Equipment not properly elevated or sealed to floor. Recommend elevating the equipment at least 4 inches above the counter top or made moveable.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed a wooden bagel cutting holder. Equipment not approved by a recognized testing agency. Wood and cut surface was warn (wood and plastic damaged). Replace holder with a approved utensil.

Observed floors in basement dry storage room to be hard to clean and in need of repairs. Floors, walls, and/or ceilings not smooth and easily cleanable.

Observed grout between tile flooring in front of handwashing sink on main service line to be in need of repair. Physical facilities not maintained in good repair.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved at the customer side of the front counter. Provide a coved base finish to allow floors to be easily cleaned.

Floors under ice bin at the end of the front service line was not easily cleanable. Floors, walls, and/or ceilings not smooth and easily cleanable.

Observed walls behind the chest freezers in the basement was not smooth and easily cleanable. Install a proper wall material to ensure easy cleaning of the storage area.

Notes:
1) Noncommercial beverage cups are to be phased out when approved utensils are received.
2) Whip cream containers shall have lid/caps on the dispensing tops to protect the food contact surfaces from contamination.
3) Bacon is cooked and transported from Lazazia Restaurant. Provide a procedure for review on how the bacon is cooked and transported. Provide the Processor registration details for review at the time of inspections.
4) If an food employee has a expose wound, wound shall be required to be covered with a impermeable bandage regardless of the wound location.
5) Dirty knives shall not be placed back on to the clean knife storage magnet. Recommend move the magnet device to a safer location.
6) Loose pony tails shall be maintained (hairnets or tucked into hats).
7) Cream and milk holding at the customer self service seems to working within the requirements of the food code.
8) Juicer source will be reviewed with ODA and feedback will be provided if anything is needed.
9) Ensure wet wiping cloth solution is replaced as often as needed to maintain proper concentration per manufactures label.
10) Residential shelving is not acceptable with in the warewashing and food preparation areas.
11) Perforated bags of cut leaf greens (if considered RTE) must be protected from contamination once removed from box. Recommend moving the bags or putting the leafy greens into a food grade container.
12) If boxes of cut leafy greens are not labeled "washed" or "Ready to Eat", produce shall be washed properly before sold to customer. Recommend requesting documentation from supplier regarding RTE status of all produce considered received RTE.

Overall, FSO was organized in maintained clean. Manager was able to demonstrate knowledge and was able to answer many questions correctly. Manager was actively responding to issues observed at the time of inspection. All violations and notes documented will be rechecked at the next standard inspection.
