[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (blender, microwave, 2 refrigerators/freezers, crock-pot, handheld mixer, roaster) at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Unapproved equipment shall be removed or replaced with commercial equipment.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the cold holding equipment ( #2) not maintained in good working order at the time of inspection. Measured hot zones (areas above 41 degrees F) within the unit at the time of inspection.
To ensure foods are being held at the proper temperature, cold holding equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair or replace this unit.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening door without proper protection against the entry of insects and rodents at the time of inspection. Back outer door was being propped opened during lunch.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. Ensure white parachutes and other outside debris and pests are not able to enter the building.

Breakfast and Lunches were provided by an exempt child food program (Community Action Council of Portage County, Inc). The program receives prepared foods from Paige food service.
The operation provides prepackaged snacks to the students.
Employee food shall be kept separate from the operation. Recommend designating space and shelving.
The violations documented will be checked for corrective action during next standard inspection.
