Comments
This inspection was conducted per the request of the new owner/manager regarding the licensing of the facility for a Risk Level III FSO. The following must be completed in order for final approval to be provided by Licensor:
1) Provide proper lighting of 50 foot candles over food preparation areas and grill area.
2) Provide proper lighting of 20 foot candles at each hand washing sink and customer self service area.
3) Complete the install of the sneeze guard on the front line.
4) Seal spaces between the walk-in cooler and freezer and wall
5) Provide a proper coved base finish on all floor and wall junctures, Counter Top and wall junctures, etc.
6) Provide proper lighting in walk-in cooler and freezer to be 10 foot candles of light.
7) Finish the install of the soda dispensing equipment.
8) Provide proof that food employees have been trained on health policies, symptoms to report to managers, and confirmed illness to report.
9) Provide documentation the supervisor is ODH certified in food protection and each shift will have at least one person with PIC training.
10) Provide a Certificate of Occupancy from Kent Building Dept. and any final inspections.
11) A signed application (application to be provided on facility passes final inspections) and license fee for the amount of $520.
12) Seal spaces around the plumbing pipes to and from grease trap in basement.
13) Provide proper metal stem temperature measuring device.
14) Provide proper temperature measuring devices in each cold and hot holding units.
15) Provide a procedure for bodily fluid clean up and kit with proper chemicals per the procedure.
