This RFE has undergone a final pre-licensing inspection and have been approved to be licensed.
A signed application and check # 1175794 for the amount of $973.00 was received at the time inspection for License # JSMH-9M2KLM.
Okay to Operate.
License will be delivered to the location on 9/30/2014
Ware washing Machine has not been approved for use at this time. Please contact the Kent Health department once 2) and 3) have been completed.

Notes on items to be checked on at 30 Day inspection:
1) Seal all the holes and opens in all the walk-in coolers and freezers (taped ceiling in walk-in freezer and door frame. Seal the areas that has the spray foam exposed.
2) Provide an alarm on the ware washing machine to indicate when the detergent is out. (visual or audible)
3) Reconnect the detergent line that was damaged.
4) Clean the top of the ware washing machine.
5) Check final drainage install on the front cold cut display case.
6) Provide a copy of final building and fire inspection. A Certificate of Occupancy will need to be provided. Verbal approval was received from the Building and Fire Department on 9/26/2014.
7) Used Cold Cut Slicing equipment (total of 6) is currently not approved this department. Please provide documentation that the Bizerba se12 us and the Bizerba se12 D food slicing equipment is still recognized by the National Sanitation Foundation. Hobart 2712 Slicer was submitted and approved in the Facility Review. The Bizerba SE12 slicers were not approved in the Facility Review. The food slicing equipment that is present at the facility will need to be replaced with NEW equipment (approved by this department) if found not be be in proper working order and/or NSF no longer will back the equipment to meet the NSF testing standards. This will be followed up on at the 30 day inspection.
8) This location is currently not preparing rotisserie chicken at this time. Please contact the Kent Health Department if plans develop to add a rotisserie cooking equipment and new food items.
