Comments
In order for the facility to be approved to be issued a Food Service Operation License, the following must be completed or provided:
1) In the dish washing room, provide additional shelving (NSF or equivalent) over the 3 compartment sink for additional air drying surfaces.
2) Seal gaps around the FRP and plumbing behind the grease interceptor.
3) Seal around floor and Grease Interceptor junctures.
4) Provide a smooth, durable and easily cleanable for the floor drain cover under steamtable.
5) Seal around the plumbing and floor juncture behind the Combine over and convection ovens.
6) Seal around beam to the left of Combine oven.
7) Provide proper backflow prevention device on direct water connection to the combine oven and the convection ovens.
8) Seal around floor drain plumbing of the food preparation sink.
9) Caulk around all door jams (mop room and restroom).
10) Provide shatter proof lighting with in the Flavoriginal cold holding unit.
11) Seal the interior of refuse cabinet, seal around floor and cabinet juncture, provide a coved based molding around base.
12) Provide proper have washing signage at each designated hand washing sink.
13) Provide a signed application (to be provided at the final inspection).
14) Ensure all cold holding and freezers are on and holding at acceptable temperatures.
15) FSO shall be ready to operate in order to license to be issued.
