3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed wall above the three-compartment sink chipping and paint peeling. Sink shall be sealed to the wall. Observed rust on the under the drain board.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface not cleaned at the required frequency. Observed air drying rack and drainboard in need of cleaning.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Flooring under shleving and around items on the floor are not maintained clean. Ensure shelving is installed off the floor atleast 6 inches and ensure products are stored off the floor.

Comments
Bottom shelves of shelving units shall be installed up off the floor at least 6 inches for cleaning purposes.
Items on the floor shall be removed to allow for easy cleaning.
The outside dumpster shall have tight fitting lids and remain closed when not in use.
Employees that handle food shall be included in the operation's sick policy.

Recommend providing make and model prior to purchasing new cooking equipment.
The fire department shall be contacted if new cooking equipment is purchased to ensure hood is not required.

The violations documented shall be corrected and will be checked during next standard inspection.
