3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature in the three-door refrigeration unit (50 degrees F). PIC stated the refrigerate temped below 41 degrees in the morning and will pull the tcs foods until the unit can be serviced by a refrigeration company.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Food employees not wearing a hair restraint.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the three-door refrigeration unit holding foods above 41 degrees F (50). PIC removed TCS foods and placed them into the milk refrigeration unit that was holding milk below 41 degrees F at the time of inspection.

No sanitizer test kit available.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface not cleaned at the required frequency. Observed the air drying rack with an accumulation of food residue and debris.
