Critical Repeat Handwashing sink is being used for purposes other than handwashing. Observed a funnel container of lime wedges stored in the basin of the handwashing sink behind the downstairs bar at the time of inspection.
A handwashing sink may not be used for purposes other than handwashing. Discard the food and remove the funnel and other debris from the handwashing sink behind the downstairs bar.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (ice bins behind all bars).
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To protect ice from contamination, eliminate all direct connections to floor drains.

The light bulb above the ice machine was properly shielded.
Flooring throughout the facility appeared smooth, durable, and easily cleanable.
Discussed the proper use of sanitizer buckets for storing wet wiping cloths. Ensure that buckets are clearly labelled, sanitizer is prepared at a concentration that is between 200 ppm and 400 ppm for quaternary ammonia, and that in-use sanitizer is changed every 2 to 4 hours.
