Repeat Physical facilities not maintained in good repair. There are multiple areas of the facility in disrepair (damaged ceiling tiles; and the floors, walls, ceilings, and floor junctures in and around the bathroom).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items on premises. There is a large amount of old equipment on the premises (cold holding units at the deli, sandwich conveyer at the deli, deli slicer, and unnecessary items in the rear portion of the building).
