| This pre-licensing inspection was conducted per the request of the future operator. The Facility was approved to be issued a Risk Classification IV FSO license with the understanding that the following will be completed in the next 15 days: 1. Provide thermometers for all coolers. 2. Provide a Certificate of Occupancy from the Kent Building Department. Kent Fire Department and Kent Building Department both gave verbal approval for the building to be occupied at time of inspection. 3. Provide a splash guard between the front hand washing and preparation sinks. The following items may need to be resolved in the future if problems are created during operation: 1. A hot water test was conducted 7/24/18. It appears that there is enough hot water to meet demands. However, if hot water is not readily available during normal operations, additional capacity may be required. 2. Provide a mop hanger over the mop sink if current location can not be kept clean and dry. Okay to Operate. A signed application and License Fee for the amount $630.00 was provided. License will be available for pickup at the Kent City Health Department 7/25/18. |
