3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (freezer, fryers, blender, keurig, microwave, containers holding food, etc.) not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Approved commercial equipment shall be used in the FSO.
3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed cold holding equipment (3-door glass reach-in) did not contain a thermometer, or the thermometer was not correctly placed at the time of inspection.
To ensure equipment is functioning properly, a properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit. Provide a thermometer for the unit listed in the observation.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed a build-up of ice in the interior of the walk-in freezer at the time of inspection.
To prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair walk-in freezer and ensure food is not stored under leaking lines.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed the interior of the beer and food walk-in cold holding units, shelving in walk-in units, and the interior of the 3-door glass cold holding unit with an accumulation of food debris and dirt at the time of inspection.
To prevent health hazards, nonfood-contact surfaces of equipment shall be kept clean. Clean and maintain the equipment listed in the observation.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Repeat Observed single-service articles (drinking straws at bar) were not handled, displayed, or dispensed properly at the time of inspection.
To prevent contamination, single-service articles or single-use articles shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule. Provide straws that are either wrapped or a dispenser that provides proper protection.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of grease on the wall, floor, and hood system around and above the fryers at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the area around the fryers.

3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Observed refrigerated, ready-to-eat, TCS foods (opened corn beef, opened lettuce, sliced tomatoes, opened egg mixture, opened cubed ham, opened potato salad, cooked ground beef, hard boiled eggs, etc.) held refrigerated for more than 24 hours were not properly date marked at the time of inspection.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. TCS food held past 7 days shall be discarded. Please incorporate a date marking system as soon as possible.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on the floors under and around the dish machine and the condensor near the door of the beer walk-in cold holding unit at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the floors listed in the observation.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Observed an accumulation of debris in the ice scoop holder and the interior of the ice machine at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean and maintain the ice scoop holder and ice machine.
